This is a free virtual program that offers diabetes risk reduction education to people with diabetes and prediabetes.
Over the duration of the program, health care provider coaches work closely with you in a small group setting to help you break old habits, create new ones, set attainable goals, and keep you motivated.
Eligibility:
The program is delivered virtually and uses a unique approach of predictive analysis, goal setting, and personalized nudges for positive behavioral change to develop healthier habits that lead to improved health outcomes.
A big plus is you can set up your own meeting times that fit your schedule!
Participants have the support of a small group of members just like you, sharing the same challenges and working towards achieving similar goals.
The program includes education for skill development, strategies to enhance motivation toward goals, and identification of environmental and social support.
Participants will access highly engaging digital tools such as the Project Power app to monitor their progress and help them stay on track. The app includes an easy-to-use food diary, weight and activity tracker, connectivity, secure discussion forum, short animated educational videos, health care provider coach/participant communication, and automated alerts and notifications. Program participants will receive a wireless scale, activity tracker, as well as other program supplies and incentives.
